Providing One-on-One Outpatient Therapy

Orange Park Medical Center provides comprehensive outpatient rehabilitation services in our new Medical Plaza building conveniently located on the medical center campus. A variety of services are available to pediatric, adolescent, adult, and geriatric patients from our highly qualified and licensed therapists.

Services include physical therapy, occupational therapy, speech pathology including a multidisciplinary approach to pediatric therapy, audiology, and a variety of other special therapeutic services. Rehabilitation services are dedicated to improving the quality of life for patients. Working closely with patient and families, we design care plans to meet each patient’s individual goals and needs.

Physical therapy and occupational therapy are forms of health care that treat physical disorders of movement, pain, and activities of daily living. Speech pathology addresses disorders of speech, cognition, communication and swallowing difficulty.

To receive outpatient rehabilitative services will require a consultation with your physician. If your physician determines rehabilitation is the best fit for your problems, a prescription will be written. Upon your first visit, an initial evaluation will be performed to determine your specific needs and an individualized plan will be customized specifically for you.
