Orange Park - Orange Park Medical Center continues to shine when it comes to heart failure care. They are the first hospital in the entire state of Florida to receive the American Heart Association’s Get With The Guidelines - Target HF Honor Roll. This remarkable recognition is achieved through documentation of all three Target: Heart Failure care components for 50% or more of eligible patients with heart failure discharged from the hospital during one calendar quarter.
This achievement is an enhancement to Orange Park Medical Center’s existing award: Get With The Guidelines®–Heart Failure Bronze Quality Achievement Award from the American Heart Association, awarded in August 2012. The recognition signified that Orange Park Medical Center reached an aggressive goal of treating heart failure patients for at least 90 days with 85 percent compliance to core standard levels of care outlined by the American Heart Association/American College of Cardiology secondary prevention guidelines for heart failure patients.
